BWI’s Soaring Together Program provides a coordinated continuum of care designed to support children, youth and families from initial assessment through stabilization, specialized care, skill development, increasing independence and lifelong connection.
Support can change as a young person’s needs change. Rather than moving between disconnected services, the Soaring Together Program creates a pathway where care, relationships, planning and supports can continue to grow alongside them.

From stabilization to lifelong connection, each step is part of one coordinated continuum of care — allowing support to grow and change alongside each child, youth and family.

Assessment & Stabilization
30–90 days
Short-term, intensive support focused on assessment, stabilization and individualized care planning.
GOAL: Stabilize, understand each young person’s needs, and develop the best long-term plan.

Specialized Therapeutic Homes
Family-style specialized care
Safe, supportive homes providing 24/7 specialized care focused on healing, development and well-being. Individualized supports may include behavioural intervention, therapy, education, life-skills development, recreation, family connection and cultural programming.
GOAL: Healing, skill-building and emotional well-being.

Medical Support Homes
Specialized care in a home environment
Individualized, home-based support for children and youth with significant medical or complex care needs. Care may include medication administration, tube feeding, seizure management, oxygen therapy, medical equipment and nursing coordination.
GOAL: Reduce hospital stays while supporting health, stability and quality of life at home.

Transitional Living Program
A graduated pathway toward independence.
Young people gradually build independence while continuing to receive the level of support they need. The program progresses from 24/7 staff support through supported, semi-independent and independent living with outreach.
GOAL: Build confidence, stability and the skills needed for successful independent living.

Family Strengthening Program
Rebuilding connections. Strengthening families.
BWI works alongside families to strengthen relationships, build practical parenting and caregiving skills, and create safer, more stable home environments. Support is individualized to each family and may include coaching, advocacy, family visits, life-skills development and connection to community resources.
GOAL: Strengthen families, improve stability and support safe, lasting connections between children, youth, and the people that matter most to them.

Life Skills Academy
Practical skills for greater independence.
Youth build practical skills in budgeting, cooking, grocery shopping, household management, education, employment readiness and community living. Learning is hands-on and individualized to each young person’s goals.
GOAL: Build confidence, practical life skills and greater independence for adulthood.

Alumni & Aftercare Support
Lifelong Connection.
BWI’s support continues beyond formal programming. Young people can stay connected through regular check-ins, crisis support, housing assistance, employment and education support, community mentorship, cultural connection and access to BWI resources.
GOAL: Provide lifelong connection and continued support as young people build successful, independent futures.
